Bracken Tor House in Okehampton, Devon, is a 7-bed care home for younger people with learning disabilities and associated mental health issues. From Bracken Tor House, we also run Reach Out Devon – an enablement service registered for personal care – where we support younger adults in the community and in the comfort of their homes. We can also provide short term respite care to help families and loved ones take a break from their caring roles.

To the rear of the property we have a small garden with a barbecue area as well as outdoor games for residents to enjoy. Four vehicles can fit on our drive, with additional parking on the street for visitors.

We promote individualised person-centred care in a relaxed and comfortable atmosphere, encouraging independence, choice and dignity throughout all that we do.

Sarah Pyne is the Registered Manager, and together with a strong team of experienced and trained staff, works throughout the day and night to ensure that our residents’ individual care needs are met with dignity and passion and that the care home feels like a ‘home from home’.

We are able to cater for individual dietary needs and preferences including specific cultural and religious dietary requirements.

Bracken Tor offers car parking, WiFi, is pet-friendly and provides a wide range of other services, including a full activities programme which is centred around every resident’s individual choices.

The aim of Bracken Tor is to provide a safe, secure and happy home life.

We have excellent working relationships with local professionals (GPs, Community Nurses, Commissioners etc.) and we have a strong reputation for providing good quality care to our residents.

From Bracken Tor House we also run Reach Out Devon – an enablement service registered for personal care – where we support younger adults in the community and in the comfort of their homes. Our aim is to provide efficient support to enable our service users to do the things they have always wanted.

At Reach Out we want you to have maximum value and control for what you pay. We therefore provide two types of support: individual, one-to-one support or support as part of a small and matching group of people. Individual activities puts you in the driving seat, while group activities allow you to socialise and increase the amount of time getting your support. We also provide domestic support in the comfort of your own home.

We started Reach Out Devon in 2010 and ever since it has been our mission to make a difference to people with learning disabilities and mental health needs by way of stimulating them and setting the right boundaries in place. Our resolve is to keep on trying and changing until we find what works to enable every individual we support. Every person that joins us is a whole new world and we thrive on achieving results with this person.

Everyone has different needs; we are passionate about finding the right solution for him or her. We have built our reputation on making things possible, bringing the right individuals together with the right support and working very closely with the professionals involved.

Sarah Pyne has been the Registered Manager since 2011, and together with Kay Peerless, the care coordinator, and their team they have done a remarkable job in meeting the various individual needs of the people we support.

We have supported people to jobs, farm work, conservation work, with end-of-life and holidays amongst many other things.
